Job Description


We are currently seeking a dynamic Supervisor, Care Coordination to join our team in the Richmond, VA area.  The Supervisor, Care Coordination under the direction of the Director, Clinical Services, is responsible for overseeing day-to-day operations within the Clinical Department for functions handled by non-licensed clinical staff. The Supervisor, Care Coordination, provides supervision and administrative oversight to the Care Coordination and Intake and Assessment Staff.  The team is organized into operating units comprised of Intake and Assessment and Care Coordination staff, and a licensed Lead Behavioral Health Care Manager and Supervisor, Care Coordination.  The Lead Care Manager is responsible for the clinical oversight and sign off of the work of the team.  The responsibility of the Supervisor, Care Coordination, includes, but is not limited to, managing case load assignments, overseeing timeliness of work product, problem solving with staff, and managing administrative tasks such as time cards and vacation schedules. 

  • The two functions the Supervisor, Care Coordination is responsible for overseeing are:
  • Intake and Assessment Specialist, who acts as the primary point of contact during member initiation and enrollment into the program, and is subsequently responsible to complete a comprehensive assessment using standardized assessment tool/s.  The program operates as an integrated, seamless and person-centered treatment approach to assist members in managing their health care, and that process is based on the results of the comprehensive assessment and reassessment processes. After assessment completion, the member’s case transitions to a Care Coordinator for ongoing management, while remaining under the clinical guidance of a licensed Lead Behavioral Health Care Manager
  • The Care Coordinator, who acts as the single point of contact for an integrated, seamless and person-centered treatment approach to assist members in managing their health care. Responsibilities include initiating and maintaining communication with members, community based behavioral health providers, primary care physicians, medical case managers and other interdisciplinary  team members to ensure that the members’ behavioral health needs are met.   Care coordinators are also responsible to work with the member’s care team on medical and psychosocial needs and goals.  The care coordinator is responsible for the member’s Individualized Care Plan (ICP) and active engagement and facilitation of the member’s Integrated Care Team (ICT) process. 
  • The goal for the ICT process is to collaborate, either in-person or through other means, to develop and implement a person-centered Individualized Care Plan (ICP) built on the individual’s specific preferences and needs, delivering services with transparency, individualization, respect, linguistic and cultural competence, and dignity and meets the medical, behavioral, LTSS, early intervention, and social needs of members.

Position Responsibilities to include, but not limited to:

  • Oversight of the day-to-day operations and staff for a Clinical pod, including Intake and Assessment and Care Coordination Staff.
  • Ensures assigned Beacon clinical pod completes of all requirements of the program as outlined by DMAS, including meeting face-to-face with members and ICTs as necessary and appropriate, timely completion of work product, documentation standards are followed, and general adherence to all relevant policies and procedures.   
  • Partnership with the Lead Care Manager, who is responsible for the clinical oversight of the pod’s work, in managing day to day operations. 
  • Partnership with other clinical pod Supervisor’s, under the direction of the Director, Clinical Services, to ensure process and procedures are followed consistently, and the program operations run seamlessly. 
  • Responsible for training, meetings, scheduling, monitoring productivity, and overall management of the assigned Clinical pod.
  • Responsible for identifying operational issues which could impact clients and members. 
  • Participate in strategic, systems, and organizational planning of department and the company as a whole.
  • Responsible for coordinating necessary documents and responses for all external audits and data requests as they apply.
  • Interface with client as needed.
  • May serves as the liaison between Clinical Department and other Beacon departments (Claims, IT, Quality Management, Member Services, and Provider Relations/Network Development) to remediate problems or identify efficiencies.   
  • Primary lead responsible for the coordination of escalations as they relate to deliverables.
  • Performs other related duties as assigned by management.

Position Requirements:


  • Bachelor’s Degree in Health or Human Services or related discipline,
  • Master’s Degree in a Mental Health or related discipline, preferred, and,
  • At least three (3) years’ experience managing a team in a health related setting

Knowledge, Skills & Abilities:

  • At least three (3) years’ experience working in mental health services and/or directly with individuals with mental health conditions in Virginia (this may include required management experience)
  • Excellent supervisory skills, including oversight of timeliness of work product and staff efficiencies
  • Knowledge of the VA Medicaid Mental Health service delivery system
  • Ability to prioritize and manage multiple tasks simultaneously while meeting deadlines for deliverables
  • Excellent written, oral and presentation skills
  • Strong problem solving skills
  • Strong organization skills, ability to multi-task, ability to manage multiple priorities and work collaboratively within a team environment
  • Must be detail oriented; able to work independently in an ever changing environment
  • Must be able to work as a team with professionals of varied disciplines
  • Above average computer literacy
