Job Description

We are currently seeking a dynamic HAWALL CONSULT CLINICAL COORDINATOR to join our team at our office in Kapolei. The Hawaii Consult Clinical Coordinator will be responsible for care coordination with the Hawaii Consult project for Beacon Health Options. This position will work with the Hawaii Consult team to ensure standardized program implementation. Utilizes clinical judgement in working directly with families, medical clinics, psychiatrists, mental health centers and independent therapists to access appropriate behavioral health or psychiatric care. Utilizing clinical judgement to problem solve access to care and integration issues between physical and behavioral healthcare. The Hawaii Consult Clinical Coordinator is responsible for the representation of Beacon Health Options and corresponding Behavioral Health Organization in face-to-face meetings with local, county, and state level administrators at various organizations.
the Hawaii Consult Clinical Coordinator is also responsible for routine face-to-face meetings, phone and email contact with all Hawaii Consult enrolled medical practices and behavioral health therapists for integration efforts

Position Responsibilities:

  • Effectively develop and maintain working relationships with the related Beacon Hawaii teams and the Hawaii Consult team for all phases of the project.
  • Recruit and develop strong working relationships with mental health centers, behavioral health therapists, medical providers and community professionals.
  • Provide outreach, recruitment, education and coordination to primary care practices in the Hawaii Consult service area. Ensure all documentation is maintained audit ready
  • Coordinate trainings with child psychiatrists, primary care practices, behavioral health providers and educational institutions. Ensure that data is gathered for tracking purposes. Act as a liaison between primary care and behavioral health for members who need behavioral health treatment follow up. Work with the Beacon Hawaii Clinical teams, local mental health centers and other providers to ensure that appropriate services are linked.
  • Knowledgeable of and demonstrated ability to access community resources for optimal care coordination.
  • Identify and address system barriers that impede smooth and seamless care delivery for members. Work with the Beacon Hawaii Clinical and Hawaii Consult team to address barriers.
  • Develop a comprehensive understanding of Beacon Hawaii Partnerships policies & procedures, clinical criteria and benefit plan requirements.
  • Develop a comprehensive understanding of system influences on Hawaii Consult providers, families served and corresponding insurance carriers.
  • In-state travel is required to primary care and behavioral health sites, as well as to meetings on the neighboring islands.
  • Maintain confidentiality and ethical professional standards.
  • Contribute to weekly staff meetings and clinical conferences as needed.
  • Demonstrate ability to balance complex tasks and informational technology systems.
  • Assist with Beacon’s participation and support of Project ECHO in Hawaii.
  • Performs special projects and other duties as assigned and required.

Position Requirements:

Education:

  • Master’s degree or higher in a health related field and licensure as a behavioral health professional (LSW or LCSW)

Licensure:

  • Must have current, valid and unrestricted Hawaii licensure with proof upon date of hire. Re-verification will take place no less than every 3 years.

Relevant Work Experience:

  • 3 plus years of Case Management
  • 3 plus years of Behavioral Health Industry Knowledge
  • 3 plus years of HIPAA and other Regulatory Knowledge
  • Experience in providing education, strategic planning, addressing barriers and facilitating meetings with multiple system/agencies is required.
  • Experience with public speaking is preferred.

Knowledge, Skills & Abilities:

  • Educated in current principles, procedures and knowledge domains of case management based on nationally recognized standards of case management

  • Knowledge of HIPAA and other privacy regulations

  • Knowledge of client's rights, state and federal laws, and regulations, such as those relating to health services, confidentiality, and consent issues for the targeted population.

  • Ability to prioritize and manage multiple projects and assignments.

  • Ability to seek help in managing workload, when appropriate.

  • Ability to work independently, handle multiple assignments and prioritize workload.

Driving/Travel requirements:

  • Requires safely operating an insured automobile for travel to off-site locations to conduct and accomplish business related activities. Must have current and valid driver’s license, access to an automobile with current registration and no fault insurance.

  • Able to travel to off-site meetings with members, providers and community agencies. Must be able to transport self and equipment to community sites, member homes, physician offices etc. May require ability to travel to Neighbor Islands and flexibility in being available to work after hours and on weekends when needed.
