About the Position:

Beacon Health Options / Massachusetts Behavioral Health Partnership (Beacon/MBHP) is seeking a field-based Community Health Worker (CHW) in southeast Massachusetts (New Bedford area) to join our team as part of our Hospital to Housing (H2H) program.  The H2H program is an exciting initiative that helps chronically homeless high utilizers of inpatient behavioral health services access permanent housing, outpatient behavioral health and primary care, and other supportive services. 

The Community Health Worker will join a team of CHWs who play a pivotal role in this unique program, the incumbent will identify homeless Beacon/MBHP Members who are eligible for participation, assist them with accessing housing and other necessary services, and provide support in an effort to reduce hospitalizations and improve health outcomes.  The CHW will also support formerly homeless individuals who have already been housed in securing health care and other resources.

Position Responsibilities:

  • Use data extracts to identify homeless individuals who are frequently hospitalized for behavioral health reasons.
  • Conduct in-person and telephonic outreach to homeless and formerly homeless individuals. Visit individuals in inpatient behavioral health settings to establish rapport and solid working relationships.
  • Work with homeless individuals to find the permanent supportive housing placements that are best suited to individual needs. Assist with housing search, housing applications, and housing placement, among other tasks.
  • Assist homeless and formerly homeless individuals with accessing primary care, mental health care, and substance use disorder treatment. Assist individuals with navigating health care and other systems.
  • Assist individuals with applying for benefits (e.g., SSI, SSDI, SNAP) and working toward income maximization. Make referrals to other appropriate services as needed.
  • Provide support to individuals struggling with behavioral health issues and homelessness. Utilize motivational interviewing and harm reduction techniques.
  • Work with participants to develop individualized service plans.
  • Perform MassHealth eligibility status checks and troubleshoot eligibility of Members.
  • Collect and enter data into client tracking systems with accuracy and efficiency. Follow proper data collection procedures to ensure all data is communicated to Beacon/MBHP in a timely manner.
  • Act as liaison between housing providers, service providers in the community, and Beacon/MBHP. Become part of Member-centered teams that work with various providers and Beacon/MBHP to meet Member needs.
  • Participate in all departmental, team, and group/individual supervision meetings as assigned by supervisor. Attend and present at internal and external trainings and conferences as assigned by supervisor.
  • Must be flexible and able to travel to locations to perform job duties, e.g. inpatient behavioral health settings, community resource sites, Beacon/MBHP regional offices, Member homes, etc. Must be able to perform physical tasks such as street outreach to homeless individuals.
  • Adhere to all HIPAA compliance regulations.
  • This position is not authorized to transport Members in personal vehicles.
  • Assist with other duties as needed.

Position Requirements:

  • Education: Associate’s degree with relevant work experience required, Bachelor’s Degree preferred
  • Licensures: clinical licensure not required, however individuals who have completed the core competency training as defined by the Massachusetts Board of Certification of Community Health Workers will be  preferred
    • Valid driver’s license and access to a vehicle required
  • Relevant Work Experience: 2 - 4 years work experience in similar field as a community health worker, peer specialist, peer advocate, or a similar role preferred
  • Knowledge, Skills & Abilities:
    • Experience dealing with behavioral health issues, navigating the behavioral health care system, and/or navigating the homeless system in Massachusetts
    • Experience developing relationships with individuals with serious mental illness and/or substance use disorder, and a demonstrated ability to connect with and support this population
    • Comfortability with meeting Members in a variety of settings, including while they are inpatient for behavioral health or medical issues, at service providers in the community (e.g. detox facilities, methadone clinics, other providers), outdoor settings where they live and/or spend time, shelters, or in their homes.
    • Knowledge of and experience with Medicaid managed care environment, Medicaid population in Massachusetts, and community resources in Springfield area strongly preferred.
    • Ability to manage multiple priorities concurrently in a fast paced environment with excellent organizational skills
    • Ability to communicate effectively with homeless individuals, treatment providers, homeless provider staff, and managed care entity teams
    • Must be self-driven and able to function independently, as well as contribute to teams within Beacon/MBHP and externally
