Job Description


We are currently seeking a dynamic Clinical Care Liaison-Simply to join our team at our office in Tampa or Miami, FL area.  The The Clinical Care Liaison acts as a single point of contact for an integrated, seamless and person-centered treatment approach to members & our Health Plan case managers in the overall management of a member’s health care.  Responsibilities include initiating and maintaining communication with our Health Plan partner for members diagnosed with BH/SUD whereupon Beacon is not delegated Case Management responsibilities.  Responsible for being the onsite clinical liaison providing clinical knowledge and behavioral health/substance abuse expertise to our Health Plan partner.  This role can provide case screening, crisis intervention, discharge planning, care coordination to Beacon Health Options members. The goal of the Clinical Care Liaison is to collaborate on-site with our Health Plan partners to assist them in the delivery of BH/SUD services. 

Position Responsibilities:

  • Coordinate and oversee outpatient referrals for integrated medical/behavioral health services to network practitioners based on practitioner specialty and member’s geographic, cultural and language preferences.

  • Collaborate with UM clinicians in identifying and removing barriers to discharge planning from acute/diversionary/outpatient levels of care; support compliance post discharge from BH inpatient admissions.

  • Oversee inpatient member discharge plans and assist in coordinating aftercare follow up appointments.

  • Triage and refer members to Health Plan medical case managers who are admitted to BH/SUD inpatient treatment

  • Attend rounds with Health Plan as needed

  • Act as liaison between hospital staff and the member’s family/support system/providers.

  • Coordinate outpatient care for members without a BH/SUD discharge plan, link members and their families to appropriate community supports and outpatient treatment.

  • Coordinate urgent BH/SUD outpatient referrals. Track communication and correspondence for all members on caseload and maintain monthly reports on referrals and case disposition.

  • Input all member data/information obtained in the course of coordinating care into the Beacon Case Management System.

  • Maintain open lines of communication with health plan contacts to ensure coordination of care.

  • Participate, as appropriate, in case rounds with the health plan contacts to discuss complex cases.

  • Play a role in identifying trends, deficiencies, and/or barriers related to ambulatory follow up so that appropriate interventions can be designed to improve overall compliance rate; support health plan initiatives and FUH/HEDIS.

  • Point of contact for development of BH/SUD reporting for UM metrics, such as high utilizer/readmissions.

  • Provide input into the development of BH/SUD referral processes and create work flows to support integration activities between Beacon and Health Plan

  • Provide BH/SUD trainings as applicable and/or assist in coordinating appropriate BH/SUD resources to provide trainings

  • Assist members and health plan case managers with BH/SUD referral network providers and providing contact information for providers and their medical case manager/coordinator.

  • Provide telephone Crisis Intervention Services, and care coordination to determine level care based on medical necessity.

  • Recruit new practitioners to meet member needs when necessary.

  • Position Requirements:

  • Master’s degree or higher in a health related field and independent licensure as a behavioral health professional required.      Licensed Mental Health professional or Registered Nurse with a valid, active, and unrestricted license to practice at a minimum in the State of Florida.

    Years and Type of Relevant Work Experience:

    Minimum 3-5 years’ experience in Mental Health / Substance Abuse treatment with emphasis on providing direct patient care, crisis intervention and discharge Planning.
  • Familiarity with all BH/SD platforms used by Beacon UM/CM.
