Job Description


We are currently seeking a dynamic Care Manager to join our team out of Plantation, FL.  The Care Manager is responsible for triaging, assessing, and providing case management services to members referred to Case Management who may be high risk, high utilizing, at significant clinical risk, or under-utilizing services. Case Management duties will be telephonic or onsite at provider facilities, community based treatment centers, etc. 

Position Responsibilities:

  • Identification of all assigned ICM (Intensive Case Management), Complex and Case Management (CM) members
  • Independent assessment and treatment planning of members enrolled in ICM, CCM/CM.
  • Managing, coordinating care, tracking, and reporting of all assigned ICM, CCM/CM members
  • Assistance in collection of outcomes information, annual analysis, and other reporting and initiatives
  • Coordinate Outpatient care for members without a discharge plan, link members and their families to appropriate community supports and Outpatient treatment
  • Regular collaboration and coordination with member’s treatment team: providers (Inpatient and Outpatient), state agencies, caretaker/guardian, health plan, natural supports, etc. to support treatment and identify and remove any barriers to care.
  • Attendance and participation in rounds with the health plan (joint, medical, etc.) to help assist in coordinating care and discuss complex co-morbid cases
  • Maintaining documentation expectations for NCQA accreditation
  • Respond to urgent calls
  • Telephonic and face to face collaboration with members
  • Treatment record reviews
  • Attendance at community clinical meetings and systems meetings as needed
  • Inputs all member data/information obtained in the course of coordinating care into the Case Management System.
  • Plays a vital role in identifying trends, deficiencies, and/or barriers related to ambulatory follow up so that appropriate interventions can be designed to improve overall compliance rate.
  • Other duties as assigned.
  • 20% travel


Position Requirements:

Education: Master’s degree or higher preferred.


Licenses:  Licensed Mental Health professional or Registered Nurse with a valid, active, and unrestricted license to practice at a minimum in the State of Florida


Relevant Work Experience:  Minimum 5 years’ experience in Mental Health / Substance Abuse treatment with emphasis on providing direct patient care, crisis intervention and basic understanding of managed care.
