Job Description


We are currently seeking a dynamic Crisis Follow- Up Coordinator to join our team at our office in Colorado Springs, CO.  The Crisis Follow-Up Coordinator is responsible for ensuring members have appropriate and timely access to care following a crisis service.  The Aftercare Coordinator works closely with members and providers, requiring excellent customer service and communication skills.  The Crisis Follow-Up Coordinator develops a good understanding and relationship with Beacon’s provider network in order to ensure members are receiving appropriate follow up behavioral health services. The Crisis Follow-Up Coordinator strives to meet all contractual and regulatory timeframes related to the functions of the job.

Position Responsibilities:

1. Successful contact with 75% of discharged members

  •  Initial contact within 48 hours of crisis service
  • Confirmation of scheduled appointment within 7 days of discharge
  • Provide assistance with scheduling appointments if no appointments have been made (or were missed)
  • Attempt to contact member 3 times within 30 days of crisis service

2.   Successful contact with 100% of outpatient providers to ensure transition of care for members discharged from an inpatient behavioral health

  •  Initiates contact with outpatient providers to determine if a member attended their scheduled appointment
  • Assist with scheduling appointments for member and/or assist in linking member and provider
  • when necessary
  • Obtain primary source verification (PSV) of kept appointments

3.   Clear and accurate documentation of work

  •   Documentation to ensure Beacon is meeting its contract standards
  • Timely and accurate completion of aftercare appointment screens
  •  Firm understanding of clinical modules on internal information system platforms
  • Ability to use a wide variety of communication tools

Position Requirements:

Education: High School diploma or GED required.  BA degree highly preferred.

Relevant Work Experience:  1-2 years’ experience in general office and/or behavioral health environment preferred

Knowledge, Skills & Abilities:

  • Microsoft Office products fluency

  • Analytical and problem solving ability

  • Excellent written and verbal communication skills

  • Excellent customer service
