About the Position:

We are currently seeking a dynamic clinician to join our team at a co-location site with potential to expand to other community settings.  The Onsite clinician will serve as the behavioral health resource to connect patients to behavioral health care based on referrals from onsite medical clinicians. The Onsite clinician provides assessments, referrals, triage, conducts telephone assessments and in-person assessments, brief treatment when appropriate, and collects sufficient data to make appropriate referral and certification decisions for emergency/urgent referrals that require alternate levels of care and makes appropriate certification decisions.  The onsite clinician provides BH education to the onsite medical.  This position reports to the Beacon Care Services Clinical Director.

Position Responsibilities:

  • Completes in-person and telephone triage, assessments and referrals; gathers demographic and clinical information to connect patient with appropriate provider and for emergency, urgent, and routine referrals
  • Screens and assesses patients for common mental health and substance abuse disorders; facilitates patient engagement and follow-up care
  • Coordinates with other Onsite clinicians within Beacon as well as within other facilities/agencies such as primary care physicians and specialists to ensure that patient comprehensive treatment needs are met
  • Provides brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate
  • Systematically tracks treatment response and monitors patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications
  • Coordinates patient’s service needs with community agencies
  • Provides education to patient and/or onsite medical staff about Behavioral Health and/or Substance Abuse Diagnosis, medication management, self-care skills (for example stress management)
  • Supports psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment
  • Provide clinical guidance and support to assist patient in achieving their goals
  • Prepares for and participates in all care team clinical rounds and clinical meetings and as needed in person meetings at the clinic site TBD

Knowledge, Skills, and Abilities:

  • Extensive knowledge of community resources/services
  • Demonstrated ability to collaborate and communicate effectively in a team setting
  • Ability to maintain effective and professional relationships with patient and other patients of the multidisciplinary care team
  • Experience with screening for common mental health and/or substance abuse disorders and based on findings, provide biopsychosocial assessments and develop plan of care
  • Working knowledge of differential diagnosis of common mental health and/or substance abuse disorders, when appropriate
  • Ability to effectively engage patients in a therapeutic relationship, when appropriate
  • Ability to work with patients by telephone as well as in person
  • Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation)
  • Basic knowledge of psychopharmacology for common mental health disorders that is within appropriate scope of practice for type of provider filling role
  • Strong orientation to person-centered experience of care  
  • Excellent written and verbal communication skills are required
  • Knowledge and experience using management information systems, including EMR platforms, as well as word processing, and spreadsheet applications (Microsoft Word and Excel experience preferred)
  • Knowledgeable about care resources and levels of care available

Position Requirements:

  • Education: B.S. Degree in nursing or Master’s Degree in a behavioral health-related field and licensure as health professional is required.
  • Licenses: Current, valid and unrestricted license in a State or territory of the United States in a mental health field (Ph.D., LCSW/ACSW, LLP, MHC, LPC, etc.).
  • Relevant Work Experience:    Minimum of three (3) years of clinical experience in a behavioral health setting/managed care experience. Outpatient care with brief solution focused training strongly preferred.   Experience in chronic disease management and health promotion preferred.
