Job Description

ABOUT THE POSITION:
We are currently seeking a dynamic Care Coordinator to join our team at our office in Manhattan or Brooklyn, NY.  The Care Coordinator reports into the structure overseen by the Clinical Director, and provides and/or coordinates a full array of care management services for members who have been referred to/are receiving care management services. The Care Coordinator directly support the activities of the licensed Care Managers, will develop an in-depth knowledge of the resources and relationships within a particular community or area, and will perform their duties as a part of the integrated team.

Position Responsibilities:

  • Provides a vast array of care coordination services that assist members in obtaining basic supports needed to improve their likelihood of connecting to community resources including but not limited to; Health Homes, Outpatient providers, Primary Care Physicians, HCBS Services
  • Outreaches the population with the purpose of providing education surrounding their plan benefits and connection to resources
  • Engages/introduces population with a Beacon Clinicians when appropriate
  • Schedules face-to-face case management and Peer Specialist services
  • Supports developed care plan and problem solves any barriers or challenges to accessing care
  • Provides referral to community resources, support services, and treatment providers
  • Tracks participation in treatment with service providers
  • Provides referrals and contacts with culturally relevant organizations and individuals needed to help prevent or reduce inappropriate institutional care
  • Serves as a liaison and resource to plan partner’s Medical Management team and activities
  • Documents all care management activities within required time frame and in required format, including documentation of all client related activities into electronic database
  • Obtain and maintains hard copy of clients’ release of information with appropriate documentation and signatures when necessary

Position Requirements: 

Education: Bachelor Degree preferred in a human service discipline

Relevant Work Experience:   Two (2) years’ experience in health care or a managed care organization setting providing services to clients with addictions and other behavioral health conditions required

Knowledge, Skills & Abilities:  Expert and proficient in:

  • MS Word, MS Excel, MS PowerPoint, MS Outlook, and MS Project
  • Analysis and problem solving
  • Interpersonal communication
  • Verbal and written communication
  • Group presentation and public speaking
  • Collaboration and team-oriented work philosophy
