Job Description

We are currently seeking a dynamic CARE MANAGER (Non Licensed) to join our Cypress office.  The Care Manager (non-licensed) is responsible for outreach to current and potential members, and screening, tracking and managing members referred to the Beacon Care Management program. Under supervision of a licensed clinician, he/she will work with identified members who have mild/moderate impairments. Members who meet moderate/severe criteria are assisted with linkage to appropriate county Specialty Mental Health Services (SMHS). The Care Manager (non-licensed) will work collaboratively with the appropriate medical health plan and help the people we serve live their lives to the fullest potential.

Position Responsibilities:

  • Telephonic collaboration with providers and county agencies regarding members in Care Management.
  • Under supervision of a licensed care manager, report coordinate care, track and report all assigned Members in FlexCare Care Management module.
  • Works collaboratively with the health plan to coordinate a supportive environment and clear communication.
  • Establishes relationships and effectively engages through telephonic communication to obtain necessary information, facilitate care, and provides behavioral health and community referrals for the purpose of care management and coordination.
  • Meets all strict deadlines and established cycle times through effective prioritization and follow-up skills.
  • Maintains accurate information in Beacon’s clinical documentation systems as directed.
  • Assists in collection of outcomes information, annual analysis and other reporting and initiatives.
  • Collaborates with Primary Care Physician (PCP), behavioral health professionals, County personnel, and other members of the health care team, including health plan Medical Care Managers, pharmacies, community based providers and others to coordinate services and optimize the member’s ability to engage in the appropriate plan of care.
  • Develops a member-centric care plan in collaboration with the member.
  • Oversees the plan of care for each assigned member, adhering to documentation timelines.
  • Conducts an assessment of health needs, and monitor and evaluate care outcomes.
  • Responds to urgent and crisis calls
  • Adheres to all Beacon policies and procedures and standards of operations.
  • Completes all required and assigned trainings.
  • Attends all mandatory company or department meetings.
  • Reports to each scheduled shift and commence work and perform essential job function at the start of each schedule shift.
  • Displays a positive, constructive, and helpful demeanor that is conducive to a safe and respectful work environment.
  • Performs special projects and other duties as assigned and required.

Position Requirements:

Education:  Master’s Degree in a behavioral health field.

Licensures: Current, valid and unrestricted MFT-I, ASW through BBS as an intern licensure required with proof on date of hire. MFT-I, ASW intern registered with the BBS accepted. Re-verification will take place no less than every 3 years.

Note: The Clinical Manager will oversee and sign off on all of the Non-Licensed Clinical Care Manager activities including clinical decisions around care and authorization of treatment. Registration with CA BBS, preferred.

Relevant Experience: 

• Knowledge of managed care and state-specific expertise preferred.

• Demonstrated experience exercising influence and negotiation skills to effectively manage patient care and health care outcomes.
