Job Description


We are currently seeking a dynamic CARE COORDINATOR to join our team at our office in the Alameda, CA area.  The Care Coordinator is responsible for outreach, screening, tracking, and managing health plan members who are referred to the Beacon Care Management program for behavioral health. The Care Coordinator will work closely with the Beacon Clinical Managers, Team Leads and Care Manager to assist identified members with mild/moderate impairments. He/she will interact collaboratively with multiple systems to ensure appropriate linkage to services and community resources.

Position Responsibilities:

  • Foster a collaborative relationship between providers, community agencies, managed care organizations, and Beacon staff in order to ensure members receive timely and integrated access to services across the continuum of care.

  • Work with members to provide effective and efficient service coordination

  • Assist members in scheduling appointments and accessing community resources

  • Accurate and timely documentation in company database of member activities and interventions

  • Continuously expand knowledge of community resources, services and programs available to members and build ongoing relationships with these organizations to advocate for members

  • Provide administrative support to the program director

  • Other duties as assigned

Position Requirements:

Education: Bachelor’s degree strongly preferred. High school diploma or GED required.

Licensure:  N/A

Relevant Work Experience:  

  • Minimum two years of experience working with the behavioral health system and/or social services sectors.

  • Experience providing patient navigation or community health worker services is a plus.

  • Must have excellent customer service phone skills.

  • Accurate and fast computer skills.

  • Experience in customer service, preferably within a mental health related field.

Knowledge, Skills & Abilities:

  • Good customer relations skills

  • Must possess high-level written and verbal skills

  • Ability to recognize and/or anticipate the needs of customers and respond accordingly

  • Ability to multi-task,

  • Ability to learn, master, and incorporate use of variety of clinical and administrative automated systems and software into daily work.

  • Must be self-directed and resourceful.
