Job Description


We are currently seeking a dynamic Aftercare Coordinator to join our team at our office in Woburn.  The Aftercare Coordinator is responsible for ensuring members have appropriate and timely access to care following a discharge from inpatient behavioral health.  The Aftercare Coordinator works closely with members and providers, requiring excellent customer service and communication skills.  The Aftercare Coordinator develops a good understanding and relationship with Beacon’s provider network in order to ensure members are receiving appropriate follow up behavioral health services. The Aftercare Coordinator strives to meet all contractual and regulatory timeframes related to the functions of the job.

Position Responsibilities:

  • Ensure appointments meet HEDIS criteria and are scheduled within 7 days of discharge from the inpatient psychiatric unit
  • Member reminder calls, provider follow-up calls, and facilitating member mailings
  • Providing clear documentation/outreach to ensure Beacon is meeting its HEDIS/NCQA standards
  • Review and audit aftercare appointment screens in flexcare to ensure these are being completed with accurate/complete information.
  • Secure appointments for members in need of urgent and routine appointments following discharge.
  • Work directly with providers and members to secure appropriate type of appointment requested
  • Serve as a backup support for other aftercare coordinators and other administrative duties within the Aftercare department

Position Requirements:

EducationBachelor’s degree preferred.

Relevant Work Experience:   1-2 years’ experience in general office and/or behavioral health environment preferred. 

Knowledge, Skills & Abilities:

  • Experience in Word
  • Excel
  • Outlook needed
  • PowerPoint preferred
